Uterus Didelphys with Obstructed Cervix

This patient was infertile, and an ultrasound revealed a complex, abnormal appearance of the female organs. An MRI was ordered to help understand the complex anatomy. This oblique coronal FSE T2-weighted image of the pelvis reveals that the uterus is duplicated (arrows). The large, cystic mass-like area seen in the midline is a fluid-filled, dilated, obstructed cervix. Uterus didelphys is a congenital anomaly of the female pelvic organs. There is complete duplication of the uterus, cervix, and portion of the vagina. There is often a septum in the upper vagina, which can obstruct the duplicated cervix (as in this case).
