For your convenience, a list of the insurance plans we participate with is provided here. The list changes periodically, so it is important to check with us when scheduling your appointment to see if there are any changes.
We encourage all patients to confirm their coverage with their insurance carrier prior to making an appointment. Our staff can assist you in obtaining prior authorizations and/or pre-certifications as may be required by your insurance plan.
Depending on your benefits you may be responsible for plan co-payments, deductibles and co-insurance.
Co-payments, deductibles and co-insurance are to be paid at time of service.
Payment plans are offered.
